CLIENT RESPONSIBILITY:
Orofacial myology is not a service that is covered by insurance carriers. I understand that I am financially responsible for all services rendered. I understand that my $150 deposit is non-refundable due to high demand and limited scheduling.

Cancellation Policy

I understand that this appointment is non-refundable. I understand that I can only reschedule this appointment. I understand that I must reschedule within 72 hours notice or I forfeit the $150 evaluation fe

No refunds will be issued at any stage of treatment.

Therapy Interruption: If therapy must stop due to unforeseen circumstances, it may be resumed later following a new comprehensive evaluation (subject to an evaluation fee).
